Atrial fibrillation (AFib) is a heart rhythm problem where the upper chambers fire chaotically, producing an irregular, often racing heartbeat: fluttering, pounding, breathlessness, and fatigue, or sometimes no symptoms at all. It is the commonest sustained rhythm disorder, its importance is less the symptoms than the stroke risk it creates (blood pools and clots in the quivering atria), and modern management (anticoagulants, rate and rhythm control, ablation) handles it well.
What does it feel like?
Three presentations: paroxysmal (episodes that start and stop suddenly: fluttering, racing, irregular pounding with breathlessness, dizziness, chest discomfort, or anxiety, lasting minutes to days), persistent or permanent (the irregularity settles in as the new normal, often noticed as reduced stamina and breathlessness on exertion), and silent (a third or more of cases: found at a routine pulse check or when a stroke tragically announces it). The pulse tells the story: irregularly irregular, no pattern to the chaos. Triggers for episodes: alcohol, caffeine in some, illness, poor sleep, and stress.
Why does it happen?
The atria's electrical wiring frays: age is the dominant driver, with high blood pressure (the biggest modifiable one), valve disease, heart failure, overactive thyroid, heavy alcohol use (holiday heart is a real entity), obesity, sleep apnea (a major under-recognized driver), and diabetes all feeding it. Sometimes no cause emerges (lone AFib, commoner in younger and athletic people). The chaos itself is not immediately dangerous in most; the danger is the clot risk and, over years of racing, weakening of the heart muscle.
How is it managed?
- Stroke prevention first: anticoagulants (apixaban, rivaroxaban, warfarin) cut stroke risk by about two-thirds; the CHA2DS2-VASc score (age, blood pressure, diabetes, prior stroke, and friends) decides who needs them, and most people with AFib do.
- Rate control: beta-blockers, digoxin, or calcium-channel blockers slow the racing heart to a civilized pace: the default strategy for many.
- Rhythm control: restoring and holding normal rhythm with antiarrhythmic drugs or cardioversion (a brief electrical reset under sedation), favored for symptomatic and younger patients.
- Catheter ablation: keyhole procedure isolating the misfiring tissue; increasingly first-line for paroxysmal AFib, with the best rhythm outcomes.
- Fix the drivers: blood pressure control, weight loss (major effect), sleep apnea treatment, alcohol reduction or abstinence, and exercise: genuinely disease-modifying, not garnish.
When is it an emergency?
AFib becomes an emergency with: chest pain, severe breathlessness, fainting, or a heart rate that will not settle (sustained very fast rates with symptoms need same-day care); signs of stroke (face droop, arm weakness, speech difficulty: call emergency services immediately, minutes matter); and signs of heart failure (swollen legs, breathlessness lying flat). Common questions
Why does everyone focus on stroke when my symptoms are the palpitations?
Because the stroke risk is the part that can devastate, while the palpitations are the part that annoys: in AFib the atria quiver instead of contracting, blood pools and can clot, and a clot ejected to the brain causes stroke; AFib-related strokes are on average more severe than other strokes. The risk is quantified by the CHA2DS2-VASc score (age, hypertension, diabetes, prior stroke, heart failure, vascular disease, sex), and above the threshold (which most people with AFib exceed), anticoagulants reduce stroke risk by about two-thirds. Symptom treatment and stroke prevention are separate conversations, and the stroke one comes first because it protects the thing you cannot recover.
Are the new anticoagulants as good as warfarin?
The direct oral anticoagulants (apixaban, rivaroxaban, dabigatran, edoxaban) are at least as effective as warfarin at preventing AFib strokes, with lower rates of bleeding into the brain, no routine blood monitoring, and few food interactions: which is why they are now first-line for most. Warfarin still has its place (mechanical heart valves, severe kidney disease, cost), and its monitoring burden (regular INR blood tests, dietary vitamin-K consistency) is real. The adherence point matters more than the molecule: DOACs wear off within a day or two of missed doses, so they protect only when taken. Aspirin, notably, no longer counts as stroke prevention in AFib: the guidelines retired it.
Should I get the ablation?
Catheter ablation (isolating the misfiring pulmonary-vein triggers, usually by freezing or heating) has moved from last resort to genuine option: for paroxysmal AFib it keeps normal rhythm in the majority (with some needing a second procedure), it outperforms drugs for rhythm control in trials, and guidelines increasingly offer it early, even first-line, for symptomatic paroxysmal AFib, especially in younger, otherwise-healthy hearts. The honest balance: it is a keyhole procedure with small but real risks, not a guaranteed cure (recurrence happens), and anticoagulation decisions run on stroke score, not on ablation success. For long-standing persistent AFib, results are more modest. It is a conversation with an electrophysiologist, not a default.
Can I drink alcohol with AFib?
The honest evidence has hardened: alcohol is a genuine AFib driver, both acutely (holiday heart: episodes triggered by binges) and chronically (regular drinking increases AFib occurrence and progression), and a randomized trial showed that abstinence in regular drinkers with AFib substantially reduced recurrences. There is no proven safe threshold for AFib specifically. The pragmatic line: complete abstinence is the best-evidenced answer, heavy reduction is the meaningful next best, and binge drinking is the worst pattern. This is one of the few conditions where the alcohol advice is not boilerplate: it is treatment.
What is the difference between rate control and rhythm control?
Two philosophies, both legitimate: rate control accepts the irregular rhythm but slows it with tablets (beta-blockers, digoxin, calcium-channel blockers), aiming for a calm pulse and symptom relief: simpler, fewer procedures, and trials show outcomes as good as rhythm control for many (especially older, less symptomatic patients). Rhythm control actively pursues normal rhythm (antiarrhythmic drugs, cardioversion, ablation), favored when symptoms are intrusive, the patient is younger, or the heart is straining. Modern practice blends them: rate control as the floor, rhythm control layered on when symptoms or circumstances argue for it. My AFib was found by accident and I feel fine. Does it still matter?
Yes, and this is the quiet truth of AFib: a third or more of cases are asymptomatic, and the stroke risk does not care whether you feel the rhythm: silent AFib carries the same clot risk as the dramatic kind, which is why incidental findings (a smartwatch alert, a pulse check) always deserve follow-up. What the absence of symptoms changes is the symptom-driven decisions (you may need less from rate or rhythm drugs), and what it does not change is the anticoagulation question, the driver hunt (thyroid, blood pressure, sleep apnea, alcohol), and the monitoring.
