Age-related macular degeneration (AMD) is deterioration of the macula, the retina's central-vision spot: the center of the picture blurs, distorts, or fades while the edges stay: faces become hard to recognize, print hard to read, straight lines bend. It is the commonest cause of sight loss in older adults in developed countries. The dry form creeps and is managed with supplements and lifestyle; the wet form can leak and bleed fast and is controlled with regular eye injections that genuinely preserve sight.
What does it look like?
The central-vision changes: reading needing more light and bigger print, faces blurring at their centers, straight lines looking wavy or bent (door frames, text lines: the distortion sign), a gray or blank patch in the center, colors dulling, and adapting slowly when moving from bright to dim rooms. It usually affects both eyes at different rates. Peripheral vision is preserved (AMD does not cause complete blindness: it takes the center), which is why navigation stays while recognition fails. Any sudden change (new distortion, a growing central patch) is the wet form's alarm.
Why does it happen?
The macula's support layer accumulates waste deposits (drusen) and thins with age (dry AMD: nine of ten cases, progressing slowly over years), and in some the eye responds by growing fragile new vessels that leak and bleed under the retina (wet AMD: fast, in weeks). The risk stack: age (the dominant one), smoking (the strongest modifiable one, doubling or more the risk), family history, high blood pressure, obesity, and decades of sunlight. It is not caused by reading or eye strain.
What actually helps?

Am I going to go blind?
The precise, reassuring anatomy of this fear: AMD damages the macula (the center of the picture), and it does not damage the peripheral retina: so AMD alone does not produce total blindness; even advanced disease leaves the edges (navigation, daylight, movement, the room around you) while taking the center (faces, print, fine detail). That is a serious loss and also a survivable one, and the practical world of low-vision support (magnifiers, lighting, technology) is genuinely effective. The second half of the answer: the wet form's central loss is substantially preventable with prompt injections, and the dry form's creep is substantially slowable with the supplement and lifestyle package. What are the injections like, and do they work?
Anti-VEGF injections (the wet-AMD treatment) sound worse than they are: the eye is numbed with drops, cleaned, and the tiny injection takes seconds (most patients describe pressure and brief weirdness rather than pain), repeated every one to few months at a dedicated clinic, often for years. They work: the large majority of treated eyes stabilize, and a meaningful share improve, with the early-starters faring best (which is why the distortion-today-appointment-today rule exists). Do the eye vitamins actually do anything?
One specific formulation does, for one specific group: the AREDS2 formula (vitamins C and E, zinc, copper, lutein, and zeaxanthin, from the large AREDS trials) slows progression to advanced disease by roughly a quarter in people with intermediate dry AMD or advanced disease in one eye: it is for that middle group, not for healthy eyes (no prevention shown) and not for already-advanced dry AMD (nothing to slow). Ordinary multivitamins and shop-eye-health blends are not the studied formula. The beta-carotene version is avoided in smokers (lung-cancer signal), which is why the current formula uses lutein instead. Ask your eye clinic whether your stage is the one it helps.
How much does smoking matter?
More than any other choice you control: smoking is the strongest modifiable risk factor for AMD (doubling or tripling the risk of developing it, accelerating progression, and blunting treatment response), and the association is dose-and-duration dependent, which means quitting at any point bends the curve. The mechanism (oxidative damage to a retina with enormous metabolic demands) is the same one the supplements target with less potency.
