A free AI doctor that travels with you

Last updated October 18, 2026.

Sick in a hotel room three states from home? Pymander is a free AI doctor by text message. It works in every US state, with no network, no referral, and no appointment.

Pymander is a free AI doctor that travelers reach by text message from anywhere in the US, with the same number that works at home. You text your number. A trained human triage responder replies in the same thread in minutes, and an AI consultation begins there: a back-and-forth in plain English that takes your history, asks what it needs to, and tells you what it thinks and what it suggests. It already knows your history, which is more than any walk-in clinic on the road will. When a symptom needs an exam, imaging, or a lab test, it says so plainly and tells you what level of care to find. It never bills anyone. It is a medical AI system, not a licensed doctor, and it cannot prescribe or order tests. For adults 18 and over. In an emergency, call 911 or go to an emergency room.

The short version: it works the same in every state because it is a text thread, not a network. Your history travels with you, and it tells you honestly what can wait until you are home and what cannot.

Why travelers use it

Networks end at the state line; text does not. Away from home, your plan may be out of network and your doctor is a phone tree. The consult is not tied to any network, plan, or zip code. If your phone works, it works.

It already knows you. A walk-in clinic in an unfamiliar city starts from a blank clipboard. The consult carries your history, your medications, and your past threads, so the answer accounts for the you that the road clinic would never meet.

The real travel question: can this wait until I am home? That is the consult's specialty. Food poisoning or something worse, sprain or fracture, wait or go now. It gives you a calibrated answer, and when the answer is go now, it tells you what level of care to find nearby.

What you get and what you do not get

You getYou do not get
A full consult by text: what this is likely to be, how worried to be, and what to do nextPrescriptions (they legally require a licensed clinician)
A red-flag check on every consult, with direct escalation when something needs emergency or same-day careA formal diagnosis or records for a travel insurance claim
Memory between visits: your history carries forward, so each consult starts where the last one endedLabs or imaging orders (planned, with licensed clinicians reviewing)
A care note you can keep or share, viewable through a signed linkEmergency care itself. In an emergency, call 911 first

How a consult works

1. Text +1 (689) 528-6964. Say what is going on in your own words. There is nothing to install and nothing to sign up for.

2. Answer a few questions. The AI doctor takes a real history the way a physician does, asking the one question that best discriminates between the possibilities it is weighing.

3. Get your assessment and plan. What it most likely is, what to do tonight, what to watch for, and whether you need a human doctor, urgently or not. Your care note arrives as a link you can keep or share.

Sick away from home

The usual options away from home are urgent care (walk in, pay cash or out-of-network rates, wait) or an emergency room, and emergency rooms must treat emergencies regardless of insurance or ability to pay. The consult helps you choose between waiting it out, urgent care, and the ER before you commit a vacation day to a waiting room. If you might file a travel insurance claim, get seen in person: insurers want documentation from a licensed provider, which a text consult is not. Within the US the service works in all 50 states; outside the US it works wherever your phone can text, but local emergency numbers, not 911, apply abroad.

The published evidence

Pymander shows its homework. The clinical AI scored 98.2% on the USMLE, the exam human doctors take, across two independent, fully logged runs of the official sample examinations (the evals page, head-to-head methodology, benchmark working paper). On HealthBench Hard, an open-ended clinical-conversation benchmark, Pymander ran a sealed, pre-registered evaluation of 1,000 conversations and scored a mean of 0.3331 (N=999), top five against every published HealthBench Hard result. The protocol, generations, and judge grades are public and rerunnable at zenodo.org/records/22714759.

Safety routing is deterministic: a separate pipeline, not the model alone, decides when a situation escalates, and the architecture is published in full in the safety architecture working paper. It was evaluated on a 300-case red-flag suite, scoring 1.0 on the 60-case gate sample with zero under-escalation on the false-reassurance probe.

Common questions

Does it work outside my home state?

Yes. It works in all 50 US states exactly the same way, because it is a text thread, not a provider network. There is no referral, no out-of-network penalty, and no new patient paperwork. Your history and past conversations come with you automatically.

Does it work internationally?

The consult itself works wherever your phone can send a text, but two things change abroad: local emergency numbers replace 911, and the consult's guidance about where to get in-person care is US-focused. For anything serious while abroad, local emergency services come first, and travel insurance with medical coverage is worth having.

Can it help me decide between waiting, urgent care, and the ER?

Yes, and that is one of its most useful roles on the road. It takes your symptoms and history and gives you a straight read on urgency: what can safely wait until you are home, what needs an urgent care today, and what needs an emergency room now. Emergency rooms must treat emergencies regardless of insurance or ability to pay.

Will my travel insurance cover anything here?

The consult itself is free, so there is nothing to claim. If you might claim travel insurance for a medical issue, get seen in person: insurers require documentation from a licensed provider, and Pymander is a medical AI system, not a licensed doctor, so it cannot provide claim documentation.

Can it prescribe if I get sick on the road?

No. It cannot prescribe medication or order tests anywhere, including at home. If you need a prescription while traveling, it will tell you what kind of visit can provide one, and your own pharmacy can often transfer an existing prescription to a location near you.

Pymander is not a replacement for a physician and does not provide medical advice, diagnosis, or treatment.
