Alitretinoin leaves your body quickly, with a blood half-life of roughly 2 to 10 hours, so most of a dose is gone within about a day or two after your last capsule.

Because the half-life is short, the drug does not build up in your body over weeks of daily dosing, and side effects such as headache, dry lips, or flushing usually settle within days of stopping. What lasts longer is the safety window around pregnancy: if you can become pregnant, contraception and pregnancy testing requirements continue for at least one month after your final dose, and you should not donate blood during that same period. Liver enzyme and cholesterol changes can also take a few weeks of monitoring to return to your baseline. Keep in mind that in the US the oral capsule form is not FDA approved, so most people here encounter alitretinoin as a topical gel applied to the skin, which produces very low blood levels. Write down the date of your last dose so you and your clinician can count the follow-up window accurately.
