Alvimopan leaves most people's systems within about 2 to 4 days after the final dose, because its half-life is roughly 10 to 17 hours. A medicine is mostly gone after four to five half-lives, which puts alvimopan at two to four days for typical use. The course itself is short: it is given in the hospital around bowel surgery, up to 15 doses over 7 days, so it does not accumulate the way a daily long-term drug does. Only a small share of each capsule reaches your bloodstream, and most of the drug and its breakdown products leave through bile and stool. Reduced kidney or liver function can stretch clearance out somewhat. Keep a written list of your discharge medicines so your regular doctor knows exactly what you received and when it stopped.
