Is it depression or just sadness?
Last updated September 3, 2026.
Sadness ties to its cause, fluctuates, and fades while you keep functioning. Depression: low mood or lost interest and pleasure, most of the day, nearly every day, 2-plus weeks, with sleep, appetite, or energy changes and impaired function. The practical splits: duration past 2 weeks, pervasiveness (it follows you into situations that should feel good), functioning (work, parenting, hygiene), and anhedonia, the specific inability to feel pleasure, which ordinary sadness never has. Grief can look identical for weeks after a real loss and is not automatically depression; the watch point is raw, immovable grief past a couple of months, or added worthlessness and thoughts of death. On the fence is exactly what screening is for: a 2-minute PHQ-9 at any primary care or telehealth visit, and depression is genuinely treatable with therapy, medication, or both. The line that skips all triage: any thoughts of suicide or self-harm mean call or text 988 now, and an active plan or intent means the ER.
What to do
- 2-plus weeks, most days, plus function loss: get screened.
- Cannot enjoy anything: the depression signature.
- Grief is not depression unless it stays immovable for months.
- A PHQ-9 takes 2 minutes at any doctor visit.
- Any suicidal thinking: call or text 988 now.
