What are the side effects of Amivantamab-vmjw Injection?

The most common side effects of amivantamab-vmjw (Rybrevant) are infusion-related reactions, an acne-like rash, painful skin and nail changes around the fingers and toes, swelling in the face or legs, mouth sores, and fatigue. Infusion reactions are most likely with the very first dose, which is why the first infusion is split over two days and you get premedications beforehand. Skin effects tend to build over the first few weeks and are usually manageable with moisturizers, antibiotic creams or pills, and sun protection. Less common but more serious effects include lung inflammation (pneumonitis), eye problems such as keratitis, and blood clots, especially when the drug is combined with lazertinib. Your oncology team checks labs, including albumin, and reviews your skin and breathing at each visit. Report new cough, shortness of breath, fever, vision changes, or leg swelling the same day rather than waiting for your next appointment.
What to do
- Plan for the first infusions: Expect the first dose to be split across two days and to take several hours, and arrange a ride home.
- Speak up during the infusion: Tell the nurse immediately about chills, tightness in your chest, dizziness, or trouble breathing so the rate can be adjusted.
- Treat skin early: Start a thick fragrance-free moisturizer and gentle cleanser from day one, and ask about a prescription rash regimen before it becomes severe.
- Protect against sun: Use broad-spectrum SPF 30 or higher, a hat, and long sleeves, since this drug makes your skin burn more easily.
- Care for nails and mouth: Keep nails short and dry, skip aggressive manicures, and rinse with a salt and baking soda solution if sores appear.
- Call the same day for red flags: New cough, breathlessness, fever, blurred vision or eye pain, or one-sided leg swelling need urgent clinician review.
