Request an amendment in writing - HIPAA requires a response within 60 days. If denied, you can add your own statement of disagreement that stays in the record.

- Track the 60-day clock - that is the federal response window, per HHS.
- Use the disagreement statement if denied - your version stays in the record permanently.
