What is a therapeutic substitution?

Last updated September 3, 2026.

It is receiving a different medication in the same class with similar effect - one statin for another - not a generic swap, which is the same molecule and needs no approval. It happens because insurance formularies push the cheapest effective option, or because of stock or cost. The protection: a different molecule is a clinical change, so the prescriber generally must approve - the pharmacist cannot silently swap molecules. If the bottle names a different drug than expected: call the pharmacy and prescriber before taking it, ask whether it covers your indication, and know that side effects on the substitute are a legitimate reason to request the original back.
