Pleurisy is inflammation of the lung's lining (the pleura): producing a sharp, stabbing chest pain that is worst on breathing in, coughing, or laughing. What does it feel like?
The signature: a sharp, stabbing, or knife-like chest pain, usually one-sided and localized, triggered precisely by breathing in deeply, coughing, sneezing, or laughing (you find yourself breathing shallow to avoid it), sometimes spreading to the shoulder. Why does it only hurt when I breathe in?
Because the pain is mechanical: the pleura is two thin layers (one wrapping the lung, one lining the chest wall) that should glide silently past each other with every breath; inflamed, they are swollen and rough, and they grate, but only when they move: the deep breath in is the maximum excursion, so it stabs precisely on inspiration (and on the cough, sneeze, and laugh, which are forceful versions of it). The honest timeline: the sharp pain peaks over the first days and settles over about one to two weeks as the viral inflammation burns out (some twinges on deep breaths can echo for a few weeks after), and the shape of recovery matters more than the calendar: steady daily improvement is the expected course; the pain getting worse, or the breathlessness, fever, or blood arriving, changes the diagnosis. The painkillers are not masking progress: they are the treatment (they keep you breathing deeply), so take them regularly through the painful days rather than testing yourself against the pain. If you are still significantly stabbing past two weeks, that is the point for the re-examination (and possibly the chest X-ray) rather than more patience.
Why does my shoulder hurt with it?

Could it keep coming back?
A single post-viral pleurisy rarely recurs (the virus passes, the pleura heals, the story ends), and the recurrence patterns point at the underlying players worth finding: repeated pleuritic episodes (especially with joint pains, rashes, or mouth ulcers) raise the autoimmune conditions (lupus and its family, which inflame serosal linings: a blood-test conversation), the recurrent clots in the risk-profiled (the inherited clotting tendencies), and the chronic pleural thickening after significant infections.
