In short What it costs without insurance The generic changes the math If you have Medicare If you have commercial insurance If you have no coverage Common questions SourcesBrand-name Entresto (sacubitril/valsartan) costs about $717 to $753 for 60 tablets without insurance, but a generic version is now available far cheaper: one online pharmacy lists a 60-tablet fill at $8.96 to $13.21 including its dispensing fee, excluding shipping. Medicare's negotiated price for Entresto took effect January 1, 2026, and eligible privately insured patients may pay as little as $10 with a Novartis co-pay offer. What you pay depends on your coverage, pharmacy and eligibility.
What it costs without insurance
Entresto is prescribed for heart failure and is usually taken twice a day, so 60 tablets make a typical one-month supply. Without insurance, Drugs.com quotes the brand at about $717 for 60 tablets across all three strengths, and GoodRx lists an average retail price around $750 to $753 for the same fill, when we checked on October 25, 2026. These are published estimates, not a quote from your pharmacy: cash prices vary by pharmacy, strength, quantity and ZIP code, so comparing two or three pharmacies before each refill can be real money.
The generic changes the math
Unlike many brand-name heart medications, Entresto now has a generic: sacubitril/valsartan, with the same active ingredients. Cost Plus Drugs, an online pharmacy, lists it in stock in all three strengths at $8.96 to $13.21 for a 60-tablet fill, including its $5 dispensing fee and excluding shipping, when we checked its published price list on October 25, 2026. That is one pharmacy's catalog price, not a checkout guarantee, and local pharmacy cash prices may be higher. If your prescription is written brand-only or you are unsure about switching, ask your prescriber or a licensed US pharmacist whether the generic is an option for you.
If you have Medicare
Two 2026 changes matter here. First, Entresto was in the first group of drugs with a Medicare negotiated price: CMS set a maximum fair price of $295 per 30-day supply, 53% below the $628 reference price, effective January 1, 2026. That is Medicare's negotiated maximum fair price, not a guaranteed patient co-pay; your plan determines what you owe. Second, Part D now caps your total out-of-pocket drug spending at $2,100 for the year across all covered Part D drugs, excluding premiums. Our guides to the 2026 Part D changes and Medicare Savings Programs and Extra Help walk through both.
If you have commercial insurance
Novartis advertises a co-pay offer of as little as $10 for up to a 90-day supply of Entresto, but the terms are narrow: it is valid only with private insurance, not with Medicare, Medicaid, TRICARE, VA, DoD or other government coverage, not if you have no insurance, and not if your plan does not cover Entresto. The combined annual benefit limit is $4,100 per calendar year, and you pay any costs above it. Enrollment auto-renews each calendar year; confirm the current terms with the program before counting on the price. If your plan co-pay is high, also compare the generic's cash price against your co-pay; for some people the generic costs less than using insurance.
If you have no coverage
If you have no prescription coverage, the generic is the route to compare first: cash prices vary widely between pharmacies, and one online pharmacy lists a 60-tablet fill under $14 (see the generic section above). Two routes do not help here: the manufacturer's co-pay offer is not valid without insurance, and Novartis's patient assistance foundation does not currently list Entresto among the medications it supports, when we checked on October 25, 2026. Do not stop or ration heart-failure medication on your own. If cost may make you skip doses, contact your prescriber or pharmacist before you run out. Our guides to affording medication and patient assistance programs list more routes.
Common questions
Is there a generic version of Entresto?
Yes. Generic sacubitril/valsartan is available in the US, with the same active ingredients as brand-name Entresto. Cost Plus Drugs lists it in stock at $8.96 to $13.21 for 60 tablets including its $5 dispensing fee, excluding shipping, checked October 25, 2026; local pharmacy cash prices vary. Ask a licensed US pharmacist about availability and whether substitution fits your prescription.
How much is Entresto per month without insurance?
For the brand, Drugs.com quotes about $717 for 60 tablets and GoodRx lists a $750 to $753 average retail price for the same fill, checked October 25, 2026. The generic costs far less: Cost Plus Drugs lists 60 tablets at $8.96 to $13.21 including its $5 dispensing fee, excluding shipping. What did the Medicare negotiated price change?
CMS set a maximum fair price of $295 per 30-day supply for Entresto, 53% below the $628 reference price, effective January 1, 2026. This is not a guaranteed patient co-pay: your Part D plan determines your cost. The 2026 annual out-of-pocket cap is $2,100 for covered Part D drugs, excluding premiums.
