In short

With a high-deductible plan, you pay the full negotiated price for most care until you meet your deductible. That makes price a real part of your decision. Ask what a service will cost before you get it, compare the insurance price with the self-pay price, and find out whether cash payments count toward your deductible.

What a high-deductible plan means in practice

Your premium is usually lower, but you pay more yourself before the plan starts sharing costs. Preventive care is often covered before you meet the deductible, which is worth checking in your plan documents. For most other care, you pay the plan's negotiated rate until the deductible is met. Our guide on what a deductible is goes through the terms.

Compare the insurance price with the cash price

Sometimes the self-pay price is lower than what you would pay through your plan before the deductible. This is common for routine labs, imaging and generic prescriptions. Ask the provider for both prices. For a sense of typical cash prices, see Pymander's lab test prices, scan prices and medication prices.

Ask whether cash payments count

Payments you make outside your insurance may not count toward your deductible. Rules vary by plan. Call the number on your insurance card and ask. If you pay cash for something that matters to your deductible, ask whether you can submit the receipt.

Ask for a good faith estimate

If you are paying yourself, providers must give an estimate of expected costs when you ask for one before a scheduled service. See our guide on the good faith estimate.

Use a health savings account if you have one

If your plan qualifies, an HSA lets you pay for medical costs with pre-tax money. For 2026 the IRS limit on contributions is $4,400 for self-only coverage under a high-deductible plan. Compare it with an FSA in our guide on HSA vs FSA.

Do not skip care because of the deductible

If you have symptoms that could be serious, such as chest pain, trouble breathing or signs of stroke, get emergency care. Cost should not be the deciding factor. For less urgent questions, our guide on urgent care vs the ER can help you choose the right setting.
