Doxazosin: what it treats, how to take it, side effects

Doxazosin is an alpha-blocker - it relaxes the muscles of the bladder and prostate so urine flows more easily, and relaxes blood vessels so blood does too. You may know it as Cardura. Two conditions, one mechanism - and one famous first-dose rule.
What does it treat?
Benign prostatic hyperplasia - the enlarged prostate that makes urinating difficult, painful, or frequent - and high blood pressure. Untreated hypertension damages the brain, heart, blood vessels, and kidneys over time; medication plus diet, weight, exercise, no smoking, and moderate alcohol is the prevention.
How do you take it?
A tablet once a day, same time, exactly as directed; your doctor starts low and increases gradually. The first-dose rule: doxazosin can make you drowsy or dizzy, so no driving, machinery, or dangerous tasks for 24 hours after the very first dose and after every dose increase. It can also cause dizziness, lightheadedness, or fainting when you stand up too fast - most common at the start, after increases, or after a gap of more than a few days. Get out of bed slowly, feet on the floor a few minutes first; if it hits, sit or lie down, and if it does not improve, call your doctor. Missed dose: take it when you remember unless the next one is close; never double up.
Side effects to know
- Common: headache, tiredness, muscle or joint pain or weakness, runny nose, sneezing, congestion, and dizziness.
- Serious - call your doctor immediately or get emergency care: rapid, pounding, or irregular heartbeat; chest pain; rash, hives, or difficulty breathing; and a painful erection that lasts for hours.
Who should not take it?
Liver disease can rule it out. Flag chest pain, low blood pressure, cataracts, any past low-blood-pressure reaction to a medication, prostate cancer, constipation, short bowel syndrome, or intestinal narrowing or blockage. Two specific notes: adults 65 and older usually should not take doxazosin for blood pressure - safer options exist for that - and if you ever need eye surgery, the surgeon must know you take or have taken doxazosin, because it changes how the iris behaves during the operation. If you become pregnant, call your doctor.
When is it an emergency?
Chest pain, trouble breathing, or an erection that is painful and lasts for hours is emergency care - that last one is priapism, and hours matter.
