Ezetimibe: what it treats, how to take it, side effects
Last updated September 3, 2026.
Ezetimibe blocks the absorption of cholesterol in the intestine. You may know it as Zetia. Where statins turn down the liver's cholesterol factory, ezetimibe closes the gut's intake door - which is why the two are so often paired, and why ezetimibe is the usual answer when a statin alone is not enough or is not tolerated.
What does it treat?
High LDL cholesterol - alone or combined with a statin (or fenofibrate) - including the inherited forms, familial hypercholesterolemia, and the rare sitosterolemia. The goal: lower cholesterol and fats in the blood to help prevent heart disease, angina, strokes, and heart attacks.
How do you take it?
One tablet once a day, with or without food, same time daily. Keep taking it even when you feel well, and do not stop without your doctor. The diet plan travels with it - the drug is an addition to, not a replacement for, the low-cholesterol eating pattern. Missed dose: take it when you remember unless the next one is close; never double up.
Side effects to know
- Common: diarrhea, sore throat, runny nose, sneezing, and joint, back, arm, or leg pain.
- Serious - uncommon, but call your doctor immediately or get emergency help: allergic signs - hives, rash, itching, trouble breathing or swallowing, hoarseness, swelling of the face, throat, tongue, lips, or eyes; fast heartbeat; dizziness or fainting; upset stomach, extreme tiredness, unusual bleeding or bruising, lack of energy, loss of appetite, pain in the upper right part of the stomach, yellowing of the skin or eyes, flu-like symptoms; and muscle pain or weakness - the statin-combination muscle watch applies here too.
Who should not take it?
Flag liver disease - especially when combined with a statin, where the liver monitoring doubles down. Report the full medication list. If you become pregnant, call your doctor.
When is it an emergency?
Trouble breathing, facial or throat swelling, fainting, or yellowing skin or eyes is urgent care. For an overdose, call Poison Control at 1-800-222-1222. Pymander's escalation routing is built and tested specifically for this class of decision; see the safety architecture working paper.

Common questions
Why add ezetimibe if I already take a statin?
Two doors, one room: the liver makes most of your cholesterol (the statin closes that), and the gut absorbs the rest from food and bile (ezetimibe closes that). When the statin alone leaves LDL above target, adding ezetimibe is the standard, evidence-backed next step - typically another meaningful drop - rather than endlessly escalating the statin dose.
Why was I given ezetimibe instead of a statin?
Usually tolerance: some people cannot take statins - real muscle symptoms or liver signals - and ezetimibe alone gives a moderate LDL reduction with a different, gentler mechanism. The trade is magnitude: solo ezetimibe lowers LDL less than a statin does. For some patients it is the best tolerated option; for others, other non-statins join the plan.
Does ezetimibe cause the statin muscle aches?
It is not a statin and does not share the mechanism - but muscle pain or weakness is on its uncommon-serious list, and the watch matters most when it is combined with a statin, where the muscle risk is additive. The protocol is the same one: report aches at visits; pain or weakness that is pronounced or persistent gets a same-day call, especially with dark urine or exhaustion.
Why do I still need the diet with two cholesterol drugs?
Because medications cap the problem; the diet is part of the cause. MedlinePlus frames ezetimibe as used "along with diet" in every indication it lists. The drugs lower the numbers and the risk; the eating pattern shifts weight, blood pressure, and glucose that no pill reaches. It is a package, not a substitution.
Why does the statin-plus-ezetimibe combination need liver checks?
Both drugs pass through the liver, and the combination is where liver-enzyme bumps show up most. So the lab schedule tightens: baseline, then periodic panels. Between draws, the symptoms that matter: upper-right stomach pain, yellowing skin or eyes, appetite loss, unusual bleeding, flu-like exhaustion - those skip the schedule and go straight to a call.
What happens if I miss a dose of ezetimibe?
Take it as soon as you remember, unless it is almost time for the next one - then skip it and never double up. Cholesterol control is a long game measured in lab panels; one missed tablet changes nothing. Same-time-daily, ideally anchored to the statin it rides alongside.
