Podofilox: what it treats, how to take it, side effects

Podofilox (brand name Condylox) is a prescription gel or solution you apply yourself to external genital warts. It destroys wart tissue by stopping the cells from dividing, and it follows a strict cycle: 3 days on, 4 days off, repeated up to four times.
What does it treat?
It treats external genital warts caused by human papillomavirus (HPV), on the penis, vulva, or the skin around the anus. It is only for warts a clinician has diagnosed and shown you how to treat; it is not for warts inside the vagina, cervix, urethra, or rectum, which need clinician-applied treatments. It clears visible warts; it does not clear the HPV infection itself, so warts can return.
How do you take it?
Apply it twice a day, morning and evening (about 12 hours apart), for 3 consecutive days, then use nothing for 4 days. That week is one cycle, and the cycle can be repeated up to four times total until no wart tissue is visible. Use the applicator or a finger to put a small amount exactly on each wart, keep it off healthy surrounding skin, and wash hands before and after. Your prescriber should demonstrate the first application. If warts remain after four cycles, you need an alternative treatment, not more cycles. Missed dose: apply when remembered unless the next dose is near.
Side effects to know
- Common: burning, stinging, redness, pain, and itching where applied, especially during the first cycles as wart tissue breaks down.
- Serious: significant skin erosion or ulceration from over-application, severe swelling, and pain that stops sleep or urination. Widespread redness and peeling beyond the treated warts means too much is being used.
Who should not take it?
Do not use it if you have had a hypersensitivity reaction to any component. It must not go inside the vagina, urethra, or rectum, and it should not touch the eyes. It is Pregnancy Category C: use reliable birth control during treatment, and if you are pregnant or breastfeeding, this drug is generally avoided in favor of clinician-applied options. Correct diagnosis matters; new growths in the genital area deserve examination before any treatment.
When is it an emergency?
Call 911 for swelling of the face, lips, or throat or trouble breathing. Severe genital pain, inability to urinate, heavy bleeding from treated areas, or signs of infection such as pus and fever need same-day care. Why the odd 3-days-on, 4-days-off schedule?
Podofilox destroys wart tissue quickly, and the 4 rest days let the surrounding skin recover so the next cycle hits wart, not raw skin. This cycle repeats up to four times. Running it daily without breaks mainly buys you ulcerated, painful skin.
Does podofilox cure HPV?
No. It removes visible warts, but the virus stays in the skin, and warts recur in a substantial share of people, especially in the first months. Clearing warts also does not mean you cannot transmit HPV. Condoms reduce but do not eliminate transmission, and vaccination prevents the strains you have not caught.
Can I use podofilox on warts inside the vagina or anus?
No. It is only for external warts on the skin. Internal warts of the vagina, cervix, urethra, or rectum must be treated by a clinician with other methods. Applying podofilox to mucous membranes causes serious injury.
What if I get it on healthy skin?
Expect stinging and redness, and wipe it off promptly. The medicine is meant to land on wart tissue only. Using the applicator tip and small amounts keeps the border clean; widespread peeling and pain beyond the warts is a sign you are using too much.
Is podofilox safe during pregnancy?
No. It is Pregnancy Category C with embryotoxicity signals in animal data, and standard guidance is to avoid it during pregnancy and use birth control while treating. Pregnant patients with genital warts get clinician-applied options like cryotherapy instead.
How long until the warts are gone?
Many warts shrink visibly within the first one or two cycles. The full allowance is four weekly cycles; if wart tissue is still visible after that, the label directs a switch to alternative treatment, which usually means an in-office procedure like freezing.
