QVAR (beclomethasone): what it treats, how to use it, side effects

QVAR Redihaler (beclomethasone dipropionate) is a twice-daily inhaled corticosteroid for asthma maintenance in patients 4 and older. Two things set it apart from other steroid inhalers: it is breath-actuated (no hand-breath coordination, no priming) and it must NOT be used with a spacer, because its extra-fine particle mist is engineered to reach small airways on its own.
What does it treat?
Maintenance treatment of asthma as preventive therapy: ages 4 to 11 at 40 or 80 mcg twice daily, ages 12 and up at 40 to 320 mcg twice daily depending on prior therapy and severity. It controls the underlying inflammation; the label is explicit that it is not for acute bronchospasm.
How do you take it?
Two inhalations (or one, by dose) twice daily, at the same times each day. The Redihaler is breath-actuated: open the cap, breathe out away from it, seal lips on the mouthpiece, inhale deep and steady, hold about 10 seconds. No shaking, no priming, no spacer. The dose counter tells you when it is empty; discard at zero. Rinse your mouth with water and spit after each use. Missed dose: skip it; never double up. Expect control to build over 1 to 4 weeks.
Side effects to know
- Common: oral thrush (the rinse-and-spit habit prevents most of it), hoarseness, throat irritation, headache, and colds.
- Serious: adrenal suppression at high doses, reduced growth velocity in children (tracked routinely), bone thinning with long high-dose use, rare glaucoma and cataracts, immune suppression around chickenpox and measles, and rare paradoxical bronchospasm.
Who should not take it?
Not for status asthmaticus or acute attacks, and not with a beclomethasone hypersensitivity. Transfer from oral steroids needs a careful taper to protect the adrenal axis. Strong CYP3A4 inhibitors (ketoconazole, ritonavir) raise exposure. Untreated infections and recent live vaccines are mention-to-your-doctor items. When is it an emergency?

Common questions
Why can't I use a spacer with the Redihaler?
Because the device is designed around its extra-fine mist: the small particles travel deep into small airways on their own, and a holding chamber would trap them. The label says do not use a spacer, which is the opposite of the advice for older HFA inhalers. Device-specific instructions beat inhaler habits here.
What does breath-actuated mean for me?
The inhaler releases the dose when you inhale, so there is no pressing-and-breathing coordination to mess up, and no priming sprays before first use. Open, breathe out away from it, inhale deep and steady, hold. It is the most technique-forgiving steroid inhaler on the market.
How is QVAR different from Flovent?
Different steroid (beclomethasone vs fluticasone propionate), same job. QVAR's distinguishing features are the extra-fine particles reaching small airways and the no-spacer, no-priming Redihaler; Flovent HFA uses a press-and-breathe inhaler that needs priming and allows spacers. Twice-daily schedules and side-effect profiles are near-identical.
Why the mouth rinsing ritual?
Steroid film left in the mouth and throat feeds thrush and causes hoarseness. Rinsing with water and spitting after each use prevents most of it. It is 10 seconds; skipping it for weeks is how people meet oral candida.
Will it stunt my child's growth?
Inhaled steroids slow growth velocity modestly at first, about a centimeter on average, without accumulating year over year, and uncontrolled asthma does its own damage. QVAR is approved from age 4, and pediatricians track height on a curve at regular visits. That tracking is routine, not a red flag.
Can I stop it once I feel normal?
Feeling normal is the drug working. Controllers are stepped down deliberately when control is proven over months, under your prescriber's plan, not dropped when symptoms vanish. Stopping abruptly is how people relearn why they were prescribed it.
