Varenicline: what it treats, how to take it, side effects
Last updated September 3, 2026.
Varenicline blocks the pleasant effects of nicotine on the brain. You may know it as Chantix. It is the most effective single medication for quitting smoking - used alongside education and counseling, with a mood watch that every patient and family should know.
What does it treat?
Smoking cessation. It blunts the reward if you do smoke and takes the edge off the craving when you do not.
How do you take it?
After eating, with a full glass of water - starting once daily and rising to twice daily, morning and evening, over the first week as the dose builds. Three ways to use it: set a quit date and start the drug 1 week before (you may smoke that first week, but stop on the date); start the drug and quit between days 8 and 35; or use it to cut down gradually toward quitting. The prescriber picks the path with you. Missed dose: take it when you remember unless the next one is close; never double up.
Side effects to know
- Common: nausea - the big one, eased by the food-and-water rule - constipation, diarrhea, gas, abdominal pain, vomiting, heartburn, bad taste, dry mouth, appetite changes, toothache, sleep trouble, unusual dreams or nightmares, headache, low energy, back, joint, or muscle pain, and abnormal menstrual cycles.
- Serious - stop taking it and call your doctor or get help immediately: allergic signs - swelling of the face, throat, tongue, lips, gums, eyes, hands, or limbs, hoarseness, trouble swallowing or breathing; skin-reaction signs - rash, swollen, red, peeling, or blistering skin, mouth blisters; heart signs - chest pain, squeezing, or pressure, pain in one or both arms, back, neck, jaw, or stomach, shortness of breath, sweating, nausea or lightheadedness with chest pain; and slow or difficult speech.
Who should not take it?
The mood conversation comes first: some people have had behavior changes, hostility, agitation, depressed mood, and suicidal thoughts on varenicline - the drug's role is unclear because quitting smoking itself shakes mental health, but the rule is firm: stop the drug and call immediately for suicidal thoughts, new or worsening depression, anxiety, or panic, agitation, restlessness, angry or violent behavior, mania, hallucinations, or any sudden unusual change in behavior, thinking, or mood. Your family should know this list too - they may see it first. Tell your doctor about any history of depression, bipolar disorder, schizophrenia, or other mental illness. It can also cause drowsiness, dizziness, and concentration problems - no driving until you know your response. Alcohol hits harder on it. If you become pregnant, call your doctor.
When is it an emergency?
Chest pain with sweating or shortness of breath, facial swelling, blistering skin, or suicidal thinking is a 911 call - and the 988 Suicide and Crisis Lifeline (call or text 988) is free, confidential, and 24/7. For an overdose, call Poison Control at 1-800-222-1222. Pymander's escalation routing is built and tested specifically for this class of decision; see the safety architecture working paper.

Common questions
Why did my doctor ask about my mental health history before prescribing it?
Because some people on varenicline have had hostility, agitation, depressed mood, and suicidal thoughts - MedlinePlus notes the drug's role is unclear (quitting smoking alone shakes mood), but the precaution stands: report any depression, bipolar disorder, or schizophrenia history, stop the drug and call immediately for suicidal thinking or any sudden behavior change, and make sure your family knows the list. It is monitored, not feared - most people take it without mood problems.
Why do I start the pill before I quit smoking?
The drug needs a week to build up and occupy the nicotine receptors - so the standard path starts varenicline 1 week before your quit date, and you may smoke during that week. What most people notice: cigarettes start delivering less, which is the mechanism working. Alternatives exist - quitting between days 8 and 35, or gradual reduction - all legitimate, all chosen with your prescriber.
Why take it after eating with a full glass of water?
Nausea is the most common side effect by a wide margin, and the food-plus-water rule is the official mitigation - it is written into the dosing instructions for exactly that reason. If nausea still bites, the prescriber can adjust the dose. The vivid dreams and nightmares, incidentally, are normal, harmless, and frequently memorable.
Why did my doctor warn me about alcohol?
Varenicline can increase the effects of alcohol - MedlinePlus advises asking your doctor about safe use. Practically: your usual tolerance may not hold during the quit attempt, and the weeks of quitting are the worst time to discover that. Many people simply pause drinking during the course.
How long is the course - and what if I slip?
Typically 12 weeks, with a second 12 weeks for people who succeed and want to consolidate - your prescriber maps it. A slip is not a failed quit: keep taking the drug, tell your support system, and reset the date. The course and the counseling are built for iteration; the average successful quitter has slipped before. The only true failure mode is stopping everything silently.
What happens if I miss a dose of varenicline?
Take it as soon as you remember, unless it is almost time for the next one - then skip it and never double up. The receptor coverage depends on steady twice-daily dosing, so anchor it to breakfast and dinner - the same meals that tame the nausea.
