A free AI doctor when your deductible makes you the payer
Last updated October 18, 2026.
With a high-deductible plan, you pay for almost everything yourself. Pymander is a free AI doctor by text message: the consult costs nothing, and the prices come before the care.
Pymander is a free AI doctor that anyone in the US reaches by text message, and it never bills, so your deductible is irrelevant to it. You text your number. A trained human triage responder replies in the same thread in minutes, and an AI consultation begins there: a back-and-forth in plain English that takes your history, asks what it needs to, and tells you what it thinks and what it suggests. When a symptom needs an exam, imaging, or a lab test, it says so plainly, and it can share published cash prices so you can compare what that care should cost before you hand over your card. It never bills anyone. It is a medical AI system, not a licensed doctor, and it cannot prescribe or order tests. For adults 18 and over. In an emergency, call 911 or go to an emergency room.
Key facts

| Availability | 24/7, including nights, weekends, and finals week |
| Who it is for | Adults 18 and over in the US |
| What it is | A medical AI system, not a licensed doctor |
| Published evidence | 98.2% on the USMLE across two independent runs; HealthBench Hard mean 0.3331 (N=999) on a sealed, pre-registered run, top five against every published result |
The short version: a high-deductible plan makes every visit a purchase, and purchases deserve price checks. The consult is free, it never touches your plan, and it can show you published cash prices before you spend a dollar against your deductible.
Why high-deductible members use it
Until you hit the deductible, you are the insurer. A $3,000 deductible means the first $3,000 of care is yours at full price. The free consult defers the most common spend of all: the visit you booked just to find out whether you needed a visit.
Price checks before, not after. Insured patients are trained never to ask what things cost, then the EOB arrives. Pymander publishes cash prices for medications, labs, imaging, and visits, and the consult can point you to them, so you can decide where to go with the number already in hand.
It never touches your plan. No claim, no network check, no prior authorization, no surprise bill three months later. The consult is free and outside your plan entirely, which makes it the one part of your healthcare with a known price: zero.
What you get and what you do not get
| You get | You do not get |
|---|---|
| A full consult by text: what this is likely to be, how worried to be, and what to do next | Prescriptions (they legally require a licensed clinician) |
| A red-flag check on every consult, with direct escalation when something needs emergency or same-day care | A formal diagnosis or paperwork for an HSA or FSA |
| Memory between visits: your history carries forward, so each consult starts where the last one ended | Labs or imaging orders (planned, with licensed clinicians reviewing) |
| A care note you can keep or share, viewable through a signed link | Emergency care itself. In an emergency, call 911 first |
How a consult works
1. Text +1 (689) 528-6964. Say what is going on in your own words. There is nothing to install and nothing to sign up for.
2. Answer a few questions. The AI doctor takes a real history the way a physician does, asking the one question that best discriminates between the possibilities it is weighing.
3. Get your assessment and plan. What it most likely is, what to do tonight, what to watch for, and whether you need a human doctor, urgently or not. Your care note arrives as a link you can keep or share.
When the deductible makes you the payer
High-deductible plans work best when you shop like a cash patient, because until the deductible is met, you are one. The same MRI can run $400 at one imaging center and $1,400 at another, and labs vary just as much. Pymander's cash price dataset shows what common care actually costs, and the consult can tell you which level of care your symptoms warrant, so the dollars you do spend against your deductible go to the right place the first time.
The published evidence
Pymander shows its homework. The clinical AI scored 98.2% on the USMLE, the exam human doctors take, across two independent, fully logged runs of the official sample examinations (the evals page, head-to-head methodology, benchmark working paper). On HealthBench Hard, an open-ended clinical-conversation benchmark, Pymander ran a sealed, pre-registered evaluation of 1,000 conversations and scored a mean of 0.3331 (N=999), top five against every published HealthBench Hard result. The protocol, generations, and judge grades are public and rerunnable at zenodo.org/records/22714759.
Safety routing is deterministic: a separate pipeline, not the model alone, decides when a situation escalates, and the architecture is published in full in the safety architecture working paper. It was evaluated on a 300-case red-flag suite, scoring 1.0 on the 60-case gate sample with zero under-escalation on the false-reassurance probe.

Common questions
Does using Pymander count against my deductible?
No. Pymander never bills anyone, so there is no claim, no charge, and nothing applied to your deductible or out-of-pocket maximum. It sits entirely outside your plan, which is exactly why it is useful to people who pay for most care themselves.
Why is this useful if I already have insurance?
Because with a high-deductible plan you pay full price for almost everything until the deductible is met. A free consult answers the question most people pay a visit fee for: whether this symptom needs care at all, and what level of care. That makes your plan something you use deliberately instead of by default.
Can it tell me what care should cost before I go?
Yes. Pymander publishes cash prices for hundreds of medications, labs, imaging studies, and visits, sourced from public datasets and updated regularly. The consult can point you to the relevant prices so you can compare facilities before you book, instead of finding out from the bill.
Will it ever tell me to just use my insurance?
Yes, when in-person care is genuinely needed, the consult says so plainly. High-deductible members sometimes delay care that should not be delayed because they are bracing for the cost. The consult is honest in both directions: it will tell you when waiting is reasonable and when it is not.
Can it prescribe or order tests?
No. It cannot prescribe medication or order labs or imaging, because it is a medical AI system, not a licensed doctor. When you need those, it tells you what kind of visit can provide them and what that visit should cost.
Pymander is not a replacement for a physician and does not provide medical advice, diagnosis, or treatment.
