A free AI doctor for the self-employed

Last updated October 18, 2026.

When you are the whole company, a sick day has no backup. Pymander is a free AI doctor you reach by text message: no employer plan, no appointment, no waiting room.

Pymander is a free AI doctor that self-employed people reach by text message, whatever coverage they carry or do not carry. You text your number. A trained human triage responder replies in the same thread in minutes, and an AI consultation begins there: a back-and-forth in plain English that takes your history, asks what it needs to, and tells you what it thinks and what it suggests. When a symptom needs an exam, imaging, or a lab test, it says so plainly, and it can share published cash prices so you know what that care should cost. It never bills anyone. It is a medical AI system, not a licensed doctor, and it cannot prescribe or order tests. For adults 18 and over. In an emergency, call 911 or go to an emergency room.

The short version: it works like a first medical opinion that costs nothing and takes minutes, from wherever you are working. No employer benefits, no appointment, and cash prices in the open when you need the next step.

Why self-employed people use it

Your time is the business. Freelancers and founders do not get sick pay, and half a day in a waiting room is half a day unpaid. A consult that starts in minutes, from your desk or your truck, means most questions get answered without losing the day.

Irregular income meets irregular care costs. When income moves around, a surprise $250 visit quote is a real decision. Pymander publishes cash prices for medications, labs, imaging, and visits, so the consult can tell you what it thinks and what the next step should cost before you commit to it.

No HR department, no one to ask. Employees get a nurse line and a benefits portal. You get a search engine. A free consult with memory of your history is the closest thing to having someone to ask, and it is available at whatever hour your work actually ends.

What you get and what you do not get

You getYou do not get
A full consult by text: what this is likely to be, how worried to be, and what to do nextPrescriptions (they legally require a licensed clinician)
A red-flag check on every consult, with direct escalation when something needs emergency or same-day careA formal diagnosis or a doctor's note for a client
Memory between visits: your history carries forward, so each consult starts where the last one endedLabs or imaging orders (planned, with licensed clinicians reviewing)
A care note you can keep or share, viewable through a signed linkEmergency care itself. In an emergency, call 911 first

How a consult works

1. Text +1 (689) 528-6964. Say what is going on in your own words. There is nothing to install and nothing to sign up for.

2. Answer a few questions. The AI doctor takes a real history the way a physician does, asking the one question that best discriminates between the possibilities it is weighing.

3. Get your assessment and plan. What it most likely is, what to do tonight, what to watch for, and whether you need a human doctor, urgently or not. Your care note arrives as a link you can keep or share.

Building something without benefits

If you buy your own coverage, the marketplace at healthcare.gov is where individual plans and income-based subsidies live, and it is worth checking during open enrollment. Whatever plan you land on, or none, the consult stays free: it does not bill, so your deductible, network, and premium are irrelevant to it. The part of healthcare the self-employed most often skip, the first opinion about whether something needs care at all, is the part that costs nothing here.

The published evidence

Pymander shows its homework. The clinical AI scored 98.2% on the USMLE, the exam human doctors take, across two independent, fully logged runs of the official sample examinations (the evals page, head-to-head methodology, benchmark working paper). On HealthBench Hard, an open-ended clinical-conversation benchmark, Pymander ran a sealed, pre-registered evaluation of 1,000 conversations and scored a mean of 0.3331 (N=999), top five against every published HealthBench Hard result. The protocol, generations, and judge grades are public and rerunnable at zenodo.org/records/22714759.

Safety routing is deterministic: a separate pipeline, not the model alone, decides when a situation escalates, and the architecture is published in full in the safety architecture working paper. It was evaluated on a 300-case red-flag suite, scoring 1.0 on the 60-case gate sample with zero under-escalation on the false-reassurance probe.

Common questions

I am self-employed. Do I need a health plan to use this?

No. Pymander is free and never bills, so it works the same whether you have a marketplace plan, a high-deductible plan, or nothing. If you are shopping for coverage, individual plans and subsidies live at healthcare.gov; the consult does not sell or recommend plans, it is care, not insurance.

Is it fast? I cannot lose a workday.

The consult starts in minutes by text and moves at the speed of the thread. Most of what people lose days over, the "is this anything" question, gets answered without leaving your desk. When the answer is that you genuinely need in-person care, it tells you that plainly rather than letting you guess.

Can it help me decide if a symptom is worth paying cash for?

Yes. That is one of its best uses. The consult tells you what your symptoms most likely mean and what level of care they warrant, and Pymander publishes cash prices for common medications, labs, imaging, and visits, so "is this worth $300" becomes an informed decision instead of a gamble.

Does it work with my high-deductible or marketplace plan?

There is nothing to work with: it never bills, so no plan is involved. That makes it useful alongside any coverage, especially high-deductible plans where you pay everything before the deductible. The free consult and the published cash prices sit entirely outside your plan.

Can it prescribe, or write a note for a client?

No. It cannot prescribe, order tests, or produce formal documentation, because it is a medical AI system, not a licensed doctor. When you need those, the consult tells you what kind of visit can provide them and what that visit should cost.

Pymander is not a replacement for a physician and does not provide medical advice, diagnosis, or treatment.
