A free AI doctor when you have no health insurance

Last updated October 18, 2026.

No health insurance changes nothing here. Pymander is a free AI doctor you reach by text message: no plan, no bill, no appointment. You text, the consult begins.

Pymander is a free AI doctor that anyone in the US reaches by text message, whether or not they have health insurance. You text your number. A trained human triage responder replies in the same thread in minutes, and an AI consultation begins there: a back-and-forth in plain English that takes your history, asks what it needs to, and tells you what it thinks and what it suggests. When a symptom needs an exam, imaging, or a lab test, it says so plainly. It can tell you where community clinics near you charge on a sliding scale, and it can share published cash prices so you know what a medication, lab, or imaging study should cost before you pay for it. It never bills anyone. It is a medical AI system, not a licensed doctor, and it cannot prescribe or order tests. For adults 18 and over. In an emergency, call 911 or go to an emergency room.

The short version: you do not need health insurance to use it. It is free, it never sends a bill, and it works over a standard text thread. When you need in-person care, it tells you what it should cost.

Why people without insurance use it

Nothing is billed, ever. There is no plan to check, no network to be in, no claim to deny. The consult is free at the point of use, and Pymander does not take a card number, so there is nothing to charge later.

Cash prices before you pay for care. The hardest part of paying cash is not knowing the price. Pymander publishes cash prices for hundreds of medications, labs, imaging studies, and visits in its catalog, and the consult can point you to them, so a $300 visit quote stops being a surprise and starts being a comparison.

Free does not mean shallow. The consult takes a real history and asks follow-up questions, not a symptom-checker flow chart. And because it is free, the cost of being told "this can wait, here is what to watch for" is nothing, which is exactly the answer most people without insurance are afraid they cannot afford to hear.

What you get and what you do not get

You getYou do not get
A full consult by text: what this is likely to be, how worried to be, and what to do nextPrescriptions (they legally require a licensed clinician)
A red-flag check on every consult, with direct escalation when something needs emergency or same-day careA formal diagnosis or a doctor's note for work
Memory between visits: your history carries forward, so each consult starts where the last one endedLabs or imaging orders (planned, with licensed clinicians reviewing)
A care note you can keep or share, viewable through a signed linkEmergency care itself. In an emergency, call 911 first

How a consult works

1. Text +1 (689) 528-6964. Say what is going on in your own words. There is nothing to install and nothing to sign up for.

2. Answer a few questions. The AI doctor takes a real history the way a physician does, asking the one question that best discriminates between the possibilities it is weighing.

3. Get your assessment and plan. What it most likely is, what to do tonight, what to watch for, and whether you need a human doctor, urgently or not. Your care note arrives as a link you can keep or share.

If you are uninsured

About 1 in 10 US adults under 65 has no health insurance, and most of the friction of that is priced in before care even starts: not knowing what a visit costs, not knowing if the symptom is worth the cost, not knowing where the sliding-scale clinics are. A free consult removes the first layer of that. If you need in-person care, community health centers charge on a sliding scale based on income (find one at findahealthcenter.hrsa.gov), and emergency rooms must treat emergencies regardless of insurance or ability to pay. Pymander's cash price dataset shows what common medications, labs, imaging, and visits cost when you pay cash.

The published evidence

Pymander shows its homework. The clinical AI scored 98.2% on the USMLE, the exam human doctors take, across two independent, fully logged runs of the official sample examinations (the evals page, head-to-head methodology, benchmark working paper). On HealthBench Hard, an open-ended clinical-conversation benchmark, Pymander ran a sealed, pre-registered evaluation of 1,000 conversations and scored a mean of 0.3331 (N=999), top five against every published HealthBench Hard result. The protocol, generations, and judge grades are public and rerunnable at zenodo.org/records/22714759.

Safety routing is deterministic: a separate pipeline, not the model alone, decides when a situation escalates, and the architecture is published in full in the safety architecture working paper. It was evaluated on a 300-case red-flag suite, scoring 1.0 on the 60-case gate sample with zero under-escalation on the false-reassurance probe.

Common questions

Do I need health insurance to use Pymander?

No. Pymander is free for every user, and it never bills anyone, so insurance status changes nothing. There is no card on file, no claim, no explanation of benefits, and no surprise bill later. The same is true whether you have a plan, are between plans, or have never had one.

Is it really free? What is the catch?

The consult is free. There is no subscription, no card required, and no upsell inside the thread. When your situation needs paid care, like a lab, imaging, a medication, or an in-person visit, the consult tells you and can share published cash prices so you can compare before you spend.

What can it actually do for me if I cannot afford care?

It gives you a free medical consultation: what your symptoms most likely mean, what self-care is reasonable, what warning signs mean you should not wait, and what level of care is worth paying for. It can point you to community health centers with sliding-scale fees and to published cash prices, so any money you do spend goes further.

Where can I get free or low-cost in-person care?

Federally funded community health centers charge on a sliding scale based on income, and nobody is turned away for inability to pay. You can find one near you at findahealthcenter.hrsa.gov. Emergency rooms must evaluate and stabilize emergencies regardless of insurance. Pymander's consult can help you figure out which level of care your situation actually needs.

Can it prescribe or order labs?

No. Pymander cannot prescribe medication, order labs or imaging, or provide a formal diagnosis. It is a medical AI system, not a licensed doctor. When your situation needs any of those, the consult tells you plainly and points you to where to get it at a known price.

Pymander is not a replacement for a physician and does not provide medical advice, diagnosis, or treatment.
