What is Afamitresgene Autoleucel Injection used for?

Afamitresgene autoleucel (brand name Tecelra) is a one-time engineered T-cell therapy used to treat advanced synovial sarcoma, a rare soft tissue cancer, in adults who have already received chemotherapy. It is used when the cancer cannot be removed with surgery or has spread to other parts of the body. Two lab results decide whether you can receive it: your tumor has to make a protein called MAGE-A4, and your blood has to carry a specific HLA-A*02 tissue type. Your own T cells are collected through apheresis, modified in a lab to target the tumor, and given back to you as a single infusion after a short course of lymphodepleting chemotherapy. It is given only at certified treatment centers, because you need close monitoring for cytokine release syndrome, low blood counts, and infection in the weeks that follow. A sarcoma specialist orders the testing and decides whether this therapy fits your situation.
What to do
- Confirm the testing first: Ask your oncologist whether your MAGE-A4 tumor result and HLA-A*02 blood typing have been done, since both are required before treatment can be planned.
- Plan for a certified center: This therapy is given only at authorized treatment sites, so ask for a referral early and plan travel and time away from home.
- Know the timeline: Cell collection, manufacturing, lymphodepleting chemotherapy, and the infusion happen over several weeks, and it is a single dose rather than an ongoing prescription.
- Stay near the center after infusion: Plan to remain close by for the monitoring period your team specifies and have someone with you who can drive and watch for changes.
- Report fever right away: Fever, chills, trouble breathing, confusion, dizziness, or unusual bruising after infusion need same-day contact with your treatment team.
- Keep your wallet card: Carry the patient card you are given so any clinician who treats you knows you received a genetically modified T-cell therapy.
