When can I kiss someone after strep throat?

In short

You can usually kiss again once your fever is gone and you have been on the right antibiotic for at least 12 to 24 hours. The CDC uses 12 to 24 hours as the point where most people can go back to school or work, and MedlinePlus says most people stop spreading strep after 24 to 48 hours of antibiotics, so waiting a full day or 2 is the safer choice for something as close as kissing. Feeling better is not the signal: the fever and the antibiotic clock are. Keep taking every dose for the full course, usually 10 days, even after you feel fine.

Why is it 12 to 24 hours and not when I feel better?

Strep throat is a bacterial infection that spreads through saliva and fluids from the nose. Kissing, shared cups and shared forks all move saliva, so they all count.

The CDC says treatment with the right antibiotic for 12 hours or longer limits how well you can pass strep to others. It tells people to stay home from school, work or daycare until they have no fever and at least 12 to 24 hours have passed since their first dose. MedlinePlus puts it a little longer: most people can spread strep until they have been on antibiotics for 24 to 48 hours.

Those sources do not agree on one exact hour, so here is a practical way to use them. Treat 12 hours as the earliest point, not the goal. For kissing, which is closer contact than sitting in a classroom, wait at least 24 hours on antibiotics and until your fever is gone without fever medicine.

  • Not yet: you still have a fever, or you started antibiotics less than 12 hours ago
  • Earliest point: no fever and 12 to 24 hours of antibiotics
  • Safer for kissing: no fever and a full 24 hours or more of antibiotics

What about sharing drinks, forks and vapes?

The CDC advises not sharing cups, utensils or bites of food with someone who is sick. Use the same timeline as kissing: no sharing until you are past the 24 hour mark with no fever.

MedlinePlus also says to get a new toothbrush after 2 or 3 days of antibiotics, but before you finish the course. If you share a cup by the sink with a partner or roommate, keep your toothbrush apart from theirs.

  • Water bottles, cups, straws and cans
  • Forks, spoons and bites off the same plate
  • Vapes, joints and anything else that goes mouth to mouth
  • Toothbrushes: replace yours after 2 or 3 days on antibiotics

Why do I have to finish the antibiotics if I feel fine?

Most strep symptoms ease within a few days, but MedlinePlus says antibiotics should be taken for 10 days even though symptoms are often gone sooner. Most CDC treatment plans for strep throat also run 10 days.

The main reason is what happens after the sore throat. The CDC says treating strep with antibiotics lowers the risk of acute rheumatic fever, an immune reaction that can inflame the heart, joints, brain and skin. It can show up about 1 to 5 weeks after a strep infection that was not treated properly. Stopping early because you feel better is how a strep infection ends up not treated properly.

Finishing the course is about protecting you. It does not change the kissing timeline, which depends on the 12 to 24 hour mark and your fever.

What if my strep is not treated?

Without antibiotics, you stay contagious longer. The sources we checked do not give one exact number of days for untreated strep, so we will not guess one. What they do say is that the 12 to 24 hour clock only starts when you start the right antibiotic.

If you have a sore throat with fever and have not been tested, skip kissing and sharing drinks until you know. A quick strep test tells you whether antibiotics are needed. Untreated strep also carries the complication risks above, including rheumatic fever.

When should you get care?

In an emergency, call 911

Get care the same day if you feel no better 24 to 48 hours after starting antibiotics, if your fever is very high, or if you have swollen glands in your neck with a rash.

Get care the same day if you have signs of dehydration, like peeing much less than usual or dark urine, or if you have a weakened immune system.

Call 911 if you have trouble breathing, cannot swallow, are drooling because you cannot swallow, make a high-pitched sound when you breathe, or are getting worse fast.

What should my partner watch for?

Strep spreads easily among people who live together or spend a lot of close time together, so your partner may already have been exposed before you knew you were sick.

Tell your partner to get tested if they develop the common signs the CDC lists. Only a test can tell strep apart from a viral sore throat.

  • Sore throat that starts fast, with pain when swallowing
  • Fever
  • Red, swollen tonsils, sometimes with white patches or streaks
  • Headache, nausea, vomiting or stomach pain, which are more common in kids
  • A rough, red rash, which can mean scarlet fever

Common questions

When can I kiss someone after strep throat?

Once your fever is gone and you have had at least 12 to 24 hours of the right antibiotic. Because MedlinePlus says most people stop spreading strep after 24 to 48 hours, waiting a full day or more is the safer choice for kissing. Keep taking the antibiotics for the full course.

Can I kiss my partner if they already have strep too?

If you both have confirmed strep and are both on antibiotics, kissing is unlikely to make either of you sicker. Still follow the 12 to 24 hour rule before kissing anyone else, and get tested if you have symptoms but no diagnosis yet.

I kissed my partner before I knew I had strep. What now?

They may have been exposed. They do not need antibiotics unless they get sick and test positive. Have them watch for a sudden sore throat, fever and swollen tonsils over the next few days and get tested if those start.

Do I need a new toothbrush after strep?

MedlinePlus recommends getting a new toothbrush after 2 or 3 days of antibiotics, before you finish the course. It is a small, cheap step, and it also helps to keep your toothbrush away from anyone else's.

Can I stop antibiotics once I feel better and can kiss again?

No. The kissing timeline and the antibiotic course are separate. Take every dose for the full course, usually 10 days, because stopping early raises the risk of complications like rheumatic fever.

Can you get strep from kissing someone who has no symptoms?

It is possible but less likely. The CDC says some people carry strep bacteria without symptoms, and these carriers are less likely to spread it to others. Most spread comes from people who are sick.
