An AI doctor you can text, like texting a friend
Last updated September 21, 2026.
Pymander is an AI doctor that works entirely by text message. No app, no account, no insurance, no waiting room. Text what is wrong; it asks the questions a doctor would ask and sends back an assessment and a plan.
Pymander is a free AI doctor you reach by text message: SMS, iMessage, or RCS, on any phone, with no app, no account, and no insurance, available 24/7 in the US for adults 18 and over. You describe what is wrong in your own words, it takes a real history one question at a time, and you get an assessment and a plan in minutes. It is a medical AI system, not a licensed doctor.

The short version: Pymander is an AI doctor that works entirely by text message - no app, no account, no insurance, free. A first consult starts the moment you text +1 (689) 528-6964. Pymander is a medical AI system, not a licensed doctor: it does not prescribe or diagnose, and it tells you plainly when you need a human clinician.
Why text instead of an app
Nothing to install. No download, no storage, no permissions, no account creation flow, no password to forget. If your phone can send a text, you already have everything a consult needs.
It works where apps fail. On a flip phone, on a cracked five-year-old Android, on one bar of signal with data turned off. SMS and iMessage are the most reliable rails a phone has.
No app-store data trail. Most health apps ship analytics and advertising SDKs that phone home before you type a symptom. A text thread has no SDK. Your consult lives in the same place as your messages with your family.
It remembers you. Most chat-based AI doctors start from zero every session. Pymander keeps your record, so next month's consult knows about the medication you mentioned today. (Do AI doctors remember you between visits?)
What a text consult looks like
1. Text +1 (689) 528-6964. Say what is going on in your own words, the way you would text a friend who happens to be a doctor.
2. Answer a few questions. The AI doctor takes a real history the way a physician does, asking the one question that best discriminates between the possibilities it is weighing - not a fixed form of checkboxes.
3. Get your assessment and plan. What it most likely is, what to do tonight, what to watch for, and whether you need a human doctor, urgently or not. Your care note arrives as a link you can keep or share.
What you get and what you do not get
| You get | You do not get |
|---|---|
| A full consult by text: what this is likely to be, how worried to be, and what to do next | Prescriptions (they legally require a licensed clinician) |
| A red-flag check on every consult, with direct escalation when something needs emergency or same-day care | A formal diagnosis with a doctor's name on it |
| Memory between visits: your history carries forward, so each consult starts where the last one ended | Labs or imaging orders (planned, with licensed clinicians reviewing) |
| A care note you can keep or share, viewable through a signed link | Emergency care itself. In an emergency, call 911 first |
When to skip the text and get a human
A text consult - this one included - is the wrong tool for emergencies. Chest pain, trouble breathing, signs of stroke, severe allergic reaction, uncontrolled bleeding: call 911 first. For everything else, the consult's job is to tell you honestly which side of that line you are on, and every consult runs that red-flag check whether you ask for it or not.
The published evidence
Pymander shows its homework. The clinical AI scored 98.2 percent on the USMLE, the exam human doctors take, across two independent, fully logged runs of the official sample examinations (the evals page, head-to-head methodology, benchmark working paper). On HealthBench Hard, an open-ended clinical-conversation benchmark, Pymander ran a sealed, pre-registered evaluation of 1,000 conversations and scored a mean of 0.3331 (N=999), top five against every published HealthBench Hard result. The protocol, generations, and judge grades are public and rerunnable at zenodo.org/records/22714759.
Safety routing is deterministic: a separate pipeline, not the model alone, decides when a situation escalates, and the architecture is published in full in the safety architecture working paper. It was evaluated on a 300-case red-flag suite, scoring 1.0 on the 60-case gate sample with zero under-escalation on the false-reassurance probe.

Common questions
Can I really text an AI doctor for free?
Yes. Text +1 (689) 528-6964 and the consult starts in the same thread. No app to install, no account to create, no card on file, no insurance checked. Pymander is a medical AI system, not a licensed doctor, and the consult is free because software scales in a way human doctors cannot.
Does it work on any phone?
Yes. Anything that can send a text message works: iPhone over iMessage or SMS, Android over SMS or RCS, and basic phones over plain SMS. There is nothing to download and nothing that needs storage, heavy data use, or a recent device.
Is texting an AI doctor private?
Consults travel over standard SMS and iMessage, the same rails as any text you send, and Pymander's handling of your record is described in the privacy policy. Texting avoids the app-store account, the email signup, and the data-broker SDKs that ship inside most health apps, but it is still a text message: share what you are comfortable sending by text.
Can an AI doctor by text prescribe medication?
No. Prescriptions legally require a licensed clinician, and any service that texts you a prescription from a bot alone is one to avoid. Pymander gives you an assessment, a plan, and a care note you can bring to your own clinician, and it tells you plainly when you need one.
What if my problem is an emergency?
Call 911 first - no text consult, ours or anyone's, is the right tool for chest pain, trouble breathing, stroke signs, severe allergic reactions, or uncontrolled bleeding. For everything else, every Pymander consult runs a deterministic red-flag check and tells you directly when something needs emergency or same-day care.
Pymander is not a replacement for a physician and does not provide medical advice, diagnosis, or treatment.
