In short

You can usually take plain Mucinex (guaifenesin only) with NyQuil Cold & Flu, because the labels share no active ingredient. Do not pair NyQuil with Mucinex DM, Mucinex Nightshift or most Mucinex Fast-Max products: they add a second dose of dextromethorphan, and Nightshift and many Fast-Max products also add acetaminophen. Check the Active Ingredients box on both packages before you take anything, and ask a pharmacist first if you have a health condition or take other medicines.

What is in NyQuil and what is in Mucinex?

The word Mucinex on a box does not tell you what is inside. The brand sells many products with very different ingredients, sometimes under almost the same name. The same goes for NyQuil. These are the active ingredients from each product's own Drug Facts label on DailyMed. Amounts can change between versions, so check your exact box:

  • NyQuil Cold & Flu liquid (per 30 mL): acetaminophen 650 mg (the drug in Tylenol), dextromethorphan 30 mg (cough suppressant), doxylamine 12.5 mg (antihistamine that makes you sleepy).
  • NyQuil Severe Cold & Flu liquid (per 30 mL): acetaminophen 650 mg, dextromethorphan 20 mg, doxylamine 12.5 mg, plus phenylephrine 10 mg (decongestant).
  • Plain Mucinex 12-hour tablet: guaifenesin 600 mg only. It is an expectorant, which loosens mucus.
  • Mucinex DM tablet: guaifenesin 600 mg plus dextromethorphan 30 mg.
  • Mucinex Fast-Max is a product line, not one formula. One Fast-Max Cold & Flu caplet has acetaminophen 325 mg, dextromethorphan 10 mg, guaifenesin 200 mg and phenylephrine 5 mg (decongestant). Another caplet sold as Fast-Max Cold & Flu has only acetaminophen and dextromethorphan. Fast-Max Severe Congestion & Cough has dextromethorphan, guaifenesin and phenylephrine, and Fast-Max Chest Congestion liquid has guaifenesin only.
  • Mucinex Nightshift Cold & Flu liquid (per 20 mL): acetaminophen 650 mg, dextromethorphan 20 mg, triprolidine 2.5 mg (antihistamine).

Which Mucinex can go with NyQuil?

Plain Mucinex, the one with guaifenesin as its single active ingredient, or another Mucinex whose label lists guaifenesin as the only active ingredient. NyQuil Cold & Flu has no guaifenesin, and plain Mucinex has none of NyQuil's ingredients, so there is no doubling up. Follow each label's own dose and timing.

One thing to know: the NyQuil label says to ask a doctor before use if you have a cough with too much phlegm (mucus). NyQuil's dextromethorphan calms coughing, while guaifenesin is meant to help you clear mucus. If your main problem is a wet, mucus-heavy cough, check with a pharmacist before you add a cough suppressant at night.

  • Plain Mucinex + NyQuil: no shared ingredient. Usually fine for adults, label rules still apply.
  • Mucinex DM + NyQuil: no. Both contain dextromethorphan.
  • Most Mucinex Fast-Max + NyQuil: no. Most Fast-Max products contain dextromethorphan, and many also contain acetaminophen. Check the box.
  • Mucinex Nightshift + NyQuil: no. Both are nighttime products with acetaminophen, dextromethorphan and a sedating antihistamine.

Why does doubling up matter?

MedlinePlus warns that cough and cold products may contain the same active ingredients, and taking them together could give you an overdose.

Acetaminophen is the biggest risk. The NyQuil Cold & Flu, NyQuil Severe and Nightshift labels, and every Fast-Max label that contains acetaminophen, say not to use it with any other drug containing acetaminophen, and warn that severe liver damage may occur if you take too much. The Nightshift label puts the maximum at 4,000 mg in 24 hours, counting other drugs that contain acetaminophen. That is a ceiling, not a target, and it includes any Tylenol you take for a headache.

Dextromethorphan is the second. MedlinePlus says taking it in large amounts can cause serious side effects or death. The overdose symptoms it lists include dizziness, fast heartbeat, hallucinations and seizures.

NyQuil (doxylamine) and Nightshift (triprolidine) each contain an antihistamine, and both labels warn that marked drowsiness may occur. Do not take two antihistamine products together, and never take two nighttime cold products at once.

Can I take Mucinex in the day and NyQuil at night?

With plain Mucinex, yes. Plain Mucinex tablets are dosed every 12 hours, and NyQuil Cold & Flu has no guaifenesin, so the two do not overlap. Stay within each label's daily limit: no more than 4 Mucinex 12-hour tablets in 24 hours, and no more than 4 doses of NyQuil Cold & Flu in 24 hours.

With Mucinex DM, most Fast-Max products or another multi-symptom Mucinex, the daytime dose and the NyQuil dose contain some of the same drugs. Spacing them out does not remove the overlap, because the same drug from both products counts toward one daily limit. If you want a day and night plan with two multi-symptom products, ask a pharmacist to check the totals before you start, or stick to one product's own directions.

When should you get care?

In an emergency, call 911

Stop taking them and see a doctor if fever gets worse or lasts more than 3 days, pain or cough gets worse or lasts more than 7 days, new symptoms appear, or a cough comes back or comes with a rash or a headache that won't go away. The labels say these could be signs of a serious condition.

Get care the same day if a sore throat is severe, lasts more than 2 days, or comes with fever, headache, rash, nausea or vomiting (the labels say see a doctor promptly), or if you get skin reddening, blisters or a rash after acetaminophen (the labels say stop and seek medical help right away).

If you think you took too much acetaminophen or dextromethorphan, call Poison Help at 1-800-222-1222 right away. The labels say quick medical attention is critical even if you do not notice any signs or symptoms.

Call 911 if someone collapses, has a seizure, has trouble breathing, or cannot be woken up.

Who should ask before taking either one?

Read the Warnings section on both boxes. These are the label cautions most likely to apply:

  • MAOI antidepressants or Parkinson's drugs: NyQuil, Mucinex DM, Nightshift and Fast-Max products with dextromethorphan all say do not use if you take an MAOI or stopped one in the last 2 weeks.
  • High blood pressure, heart disease, thyroid disease or diabetes: NyQuil Severe and Fast-Max products with phenylephrine contain a decongestant, and their labels say ask a doctor first.
  • Liver disease, glaucoma, or trouble urinating from an enlarged prostate: listed on the NyQuil and Nightshift labels as ask a doctor first.
  • Chronic cough from smoking, asthma, chronic bronchitis or emphysema: listed on both NyQuil and Mucinex labels.
  • Sedatives, tranquilizers, or warfarin: NyQuil and Nightshift say ask a doctor or pharmacist first.
  • Pregnant or breastfeeding: both labels say ask a health professional.
  • Children: plain Mucinex, Mucinex DM and Nightshift say do not use under 12. NyQuil Cold & Flu says ask a doctor for ages 4 to under 12 and do not use under 4.

Common questions

Can I take NyQuil and Mucinex together?

Plain Mucinex (guaifenesin only) has no ingredient in common with NyQuil Cold & Flu, so most adults can use both as each label directs. Mucinex DM, Fast-Max and Nightshift duplicate NyQuil ingredients, so do not combine those. Ask a pharmacist first if you have a health condition or take other medicines.

Can I take Mucinex DM with NyQuil?

No. Mucinex DM contains 30 mg of dextromethorphan per tablet, and NyQuil Cold & Flu contains 30 mg per dose. Taking both adds a second dose of the same cough suppressant. If you need NyQuil at night, plain Mucinex is the version without overlap.

Is Mucinex Nightshift the same as NyQuil?

They are close. Both have acetaminophen, dextromethorphan and a sedating antihistamine (triprolidine in Nightshift, doxylamine in NyQuil). They are alternatives, so pick one and never take both.

I already took Mucinex Fast-Max and NyQuil. What now?

Do not take any more of either. Check whether your Fast-Max box lists acetaminophen and add up the acetaminophen from both labels. Call Poison Help at 1-800-222-1222 for advice now, even if you feel well, and keep the boxes handy. Call 911 if someone has a seizure, trouble breathing, or cannot be woken up.

Can I add Tylenol, Advil or Motrin to NyQuil and Mucinex?

Not Tylenol: NyQuil already contains acetaminophen, the drug in Tylenol. Advil and Motrin are ibuprofen, which is not in NyQuil Cold & Flu or plain Mucinex, but ask a pharmacist before adding it, especially if you have a health condition or take other medicines. Never take two NSAIDs together, such as ibuprofen with naproxen (Aleve).

How long should I wait between Mucinex and NyQuil?

With plain Mucinex there is no shared ingredient, so there is no required gap between the two. Follow each product's own schedule: plain Mucinex every 12 hours and NyQuil Cold & Flu every 6 hours, with no more than 4 tablets of Mucinex or 4 doses of NyQuil in 24 hours.
