When can I have sex again after my BV medicine?Text the AI doctor
In short

Wait until you have finished your full course of BV medicine and your symptoms are gone. The CDC says to avoid sex or use condoms consistently and correctly during treatment. If you used clindamycin cream, it can weaken latex condoms and diaphragms for up to 5 days after your last dose, so count that time too. Male partners don't need treatment under current CDC guidelines.

What does the CDC say about sex during BV treatment?

The CDC 2021 STI Treatment Guidelines say women "should be advised to refrain from sexual activity or to use condoms consistently and correctly during the BV treatment regimen."

The common treatments are 7 days of metronidazole pills, 5 days of metronidazole vaginal gel, or 7 nights of clindamycin vaginal cream. Other options include single-dose secnidazole, tinidazole, or clindamycin pills or ovules. Count from the day you take or use the last dose.

The CDC BV fact sheet says to take all the medicine you were prescribed, even if your symptoms go away.

How long does clindamycin cream affect condoms?

Clindamycin vaginal products contain oil that can weaken latex. The exact time window depends on which source you read:

  • CDC guidelines: clindamycin cream "might weaken latex condoms and diaphragms for 5 days after use."
  • Clindesse label: condoms and diaphragms are "not recommended concurrently or for 5 days following treatment."
  • Cleocin vaginal cream and ovule labels: use of condoms or diaphragms "within 72 hours following treatment" is not recommended.
  • All clindamycin vaginal labels: do not have vaginal intercourse, or use tampons or douches, during treatment.

So when is it safe to have sex again?

Use the longer number if you are not sure. A simple plan:

  • Metronidazole pills or other oral treatment: wait until the course is done and symptoms are gone. Use condoms if you have sex during treatment.
  • Metronidazole gel: same as above. Finish the 5 days and wait for symptoms to clear.
  • Clindamycin cream: no vaginal sex during treatment, then do not rely on latex condoms or a diaphragm for 5 days after the last dose.
  • Clindamycin ovules: no vaginal sex during treatment, then do not rely on latex condoms or a diaphragm for at least 72 hours after.

Can I drink alcohol on metronidazole for BV?

The sources disagree here. The Flagyl (metronidazole) label says to stop drinking alcohol, and products with propylene glycol, "during and for at least three days after therapy," because cramps, nausea, vomiting, headaches, and flushing may occur.

The CDC 2021 guidelines reviewed the evidence and concluded that "refraining from alcohol use while taking metronidazole (or tinidazole) is unnecessary." Since your pharmacy label will likely follow the drug label, ask your pharmacist or prescriber before you drink.

Does my partner need to be treated?

The CDC fact sheet says "Male sex partners of women with BV do not need treatment," and the 2021 guidelines say routine treatment of sex partners is not recommended because past trials did not show it prevented BV from coming back.

Female partners are different. The CDC says BV can spread between female sex partners. If your partner has symptoms, they should get checked too.

The CDC also says all women with BV should be tested for HIV and other STIs, since having BV can raise your chances of getting other STIs.

When should you get care?

In an emergency, call 911

Get care the same day if you have fever, belly or pelvic pain, or pain with sex that is new. The CDC links untreated BV to pelvic inflammatory disease.

Get care soon if you are pregnant and have BV symptoms. The CDC recommends treatment for all pregnant women with symptoms, because BV is linked to early birth and low birth weight.

Get care the same day if you get a skin rash, blisters, or peeling after starting metronidazole. The label reports rare severe skin reactions.

Call 911 if you have trouble breathing, swelling of the face or throat, or you faint.

What if BV keeps coming back?

This is common. The CDC fact sheet says "BV can return even after treatment," and the guidelines say women should return for evaluation if symptoms recur. Follow-up is not needed if symptoms clear.

For people with repeated BV, the CDC describes options a clinician can prescribe, such as metronidazole gel twice a week for more than 3 months. It notes the benefit does not last once the suppressive treatment stops.

Things the CDC says may lower your risk:

  • Don't douche. The CDC says douching "might increase the risk for relapse."
  • Use condoms correctly every time you have sex.
  • Limit your number of sex partners.

Common questions

When can I have sex after BV treatment?

After you finish the full course and your symptoms are gone. The CDC says to avoid sex or use condoms consistently during treatment. After clindamycin cream, don't rely on latex condoms or a diaphragm for 5 days.

Can I have sex while taking metronidazole for BV?

The CDC says either avoid sex or use condoms consistently and correctly during treatment. Finishing the course before sex is the simplest choice.

Can my boyfriend give me BV back?

The CDC says how BV spreads is not fully understood, but condoms and fewer partners may lower your risk. Current CDC guidelines do not recommend treating male partners, because past trials did not show it prevented BV from coming back.

Do condoms work with clindamycin cream?

Not reliably. The CDC says it can weaken latex condoms and diaphragms for 5 days after use, and the Cleocin labels say 72 hours. Avoid relying on them during that window.

How long after metronidazole can I drink alcohol?

The Flagyl label says wait at least 3 days after your last dose. The CDC 2021 guidelines say avoiding alcohol is unnecessary. Ask your pharmacist which advice fits your prescription.
