A short list: prescribers and pharmacists treating you (including through state PDMP databases, which exist so controlled-substance fills are visible across pharmacies), your insurer for what they pay for, and anyone you authorize in writing. Not on the list: employers (pre-employment drug testing is a separate consented process) and family (no automatic access). The nuances: cash prescriptions stay out of the insurer's view but still sit in pharmacy and PDMP records, and discount cards and pharmacy apps trade some data - read what you sign up for.
