An AI doctor that remembers you
Last updated September 21, 2026.
Most AI doctors forget you the moment the chat closes. Pymander keeps your record, so every consult starts where the last one ended - your conditions, your medications, what was already ruled out.
Pymander is a free AI doctor with memory: it keeps your record between visits, so a consult next month knows about the medication you mentioned today. You reach it by text message, with no app, no account, and no insurance, 24/7 in the US for adults 18 and over. It is a medical AI system, not a licensed doctor.
Key facts

The short version: Pymander is an AI doctor that remembers you. It keeps your record between visits - conditions, medications, allergies, past consults - so you never re-explain your history to a blank chat. Free, by text message, no app or account. Pymander is a medical AI system, not a licensed doctor: it does not prescribe or diagnose, and it tells you plainly when you need a human clinician.
Why memory is the whole game
Medicine is longitudinal. A headache means something different for someone with a history of migraines than for someone with none. A symptom that has been slowly worsening for three weeks is a different problem from the same symptom appearing today. An AI that starts from zero every session cannot tell those apart; one that remembers you can.
Fewer repeated questions, better questions. Because the record carries forward, the consult skips what it already knows and spends its questions on what actually discriminates between the explanations still on the table.
Interactions get caught. When you mention a new symptom or a new supplement, the check runs against what you already take - because the record knows what that is.
You can audit it. Ask "what do you remember about me?" in any thread and it will tell you. No hidden profile.
How it compares
Most AI doctors are session-based: web chatbots and app chat windows that reset when you close them. A few are building memory under brand names, usually behind accounts and subscriptions. Pymander's memory is the default, not a tier: the first free consult starts your record, and every later consult builds on it. We publish honest side-by-sides, including the best free AI doctors compared and do AI doctors remember you between visits?
What you get and what you do not get
| You get | You do not get |
|---|---|
| A record that carries forward: each consult starts where the last one ended | Prescriptions (they legally require a licensed clinician) |
| A red-flag check on every consult, with direct escalation when something needs emergency or same-day care | A formal diagnosis with a doctor's name on it |
| Wearable context: your real sleep, heart-rate, and activity numbers in the conversation | Labs or imaging orders (planned, with licensed clinicians reviewing) |
| A care note you can keep or share, viewable through a signed link | Emergency care itself. In an emergency, call 911 first |
The published evidence
Pymander shows its homework. The clinical AI scored 98.2 percent on the USMLE, the exam human doctors take, across two independent, fully logged runs of the official sample examinations (the evals page, head-to-head methodology, benchmark working paper). On HealthBench Hard, an open-ended clinical-conversation benchmark, Pymander ran a sealed, pre-registered evaluation of 1,000 conversations and scored a mean of 0.3331 (N=999), top five against every published HealthBench Hard result. The protocol, generations, and judge grades are public and rerunnable at zenodo.org/records/22714759.
Safety routing is deterministic: a separate pipeline, not the model alone, decides when a situation escalates, and the architecture is published in full in the safety architecture working paper. It was evaluated on a 300-case red-flag suite, scoring 1.0 on the 60-case gate sample with zero under-escalation on the false-reassurance probe.

Common questions
Which AI doctor remembers you between visits?
Pymander does. It keeps a record of your conditions, medications, allergies, past consults, and what was already ruled out, and every new consult starts from that record instead of a blank chat. Most AI doctors - web chatbots and app-based ones alike - forget you the moment the session closes.
What exactly does it remember?
Your stated conditions and history, your medications and allergies, past symptoms and what they turned out to be, what you have already tried, and your preferences (for example, that you prefer to avoid a certain drug). You can ask it what it remembers about you at any time and it will tell you.
Why does memory matter in a medical AI?
Because medicine is longitudinal. A headache means something different for someone with a history of migraines than for someone with none; a new symptom lands differently against last month's consult than against a blank form. A doctor who remembers you asks fewer repeated questions, catches interactions with what you already take, and notices when something is new for you versus getting worse.
Does it connect to my wearables?
Yes. Pymander can read your wearable data (Garmin, Oura, WHOOP, Strava), so a consult about poor sleep or a racing heart starts from your actual numbers instead of your estimate of them.
Can an AI doctor that remembers me prescribe?
No. Prescriptions legally require a licensed clinician, whoever or whatever remembers you. Pymander gives you an assessment, a plan, and a care note you can bring to your own clinician - and because it remembers your record, that note reflects your whole picture, not one visit.
Pymander is not a replacement for a physician and does not provide medical advice, diagnosis, or treatment.
